Homebound Delivery Policy

Policy

Perry Cook Memorial Public Library will provide delivery and retrieval of library materials free of charge to and from eligible patrons who cannot drive due to age, injury, illness or other restriction.  Patrons who need this service on a temporary basis also qualify.  Outreach patrons must have a library card and reside within the Northmor Local School District boundaries (the Perry Cook Memorial Public Library service area).  Deliveries are limited to library materials only.  Library staff will deliver or send materials the patron specifically requests or choose items based on the patron's interests.  Outreach patrons must provide a safe and appropriate environment for staff members or volunteers who make deliveries to their homes.  Materials will be delivered in a blue book bag clearly marked "Perry Cook Memorial Library."

Staff have discretion to not enter a home, to leave a home, and/or recommend suspension of service if they feel uncomfortable at the residence for any reason.  To prepare for deliveries, please confine pets, dress appropriately, and exercise good manners during the delivery.  Should a delivery concern arise, a library supervisor will contact the patron to discuss.

Outreach patrons will not be charged for late returns, but materials which are lost or returned in severely damaged condition will be billed to the patron.

Homebound deliveries will occur on scheduled days/times, every six weeks.  Staff will contact patrons so patrons know when to expect delivery.  If a patron is not home to receive materials, library staff will contact the patron and attempt delivery materials again.  If the patron is not home to receive materials after a second attempt, the patron's delivery service will be suspended.

Any situation not specifically covered will be resolved by the Library Director and may be reviewed by the Library Board.

 

Homebound Delivery Applications:

  1. Please complete the Homebound Delivery Services application or call the library at 419-362-7181 to have an application e-mailed or mailed to you.  If you are able, you may also email the application to admin@perrycooklibrary.org .  If you do not want to complete the reading preferences section, a library staff member will choose materials for you.  There is a space on the application where you may choose whether or not the library may maintain a record of your reading history, to assist in choosing books for deliveries.
  2. If you do not have a Perry Cook Memorial card, we will mail a card application to you.  We'll need a copy of your photo ID returned with your completed library card application.  Your library card will be mailed to you.
  3. Once your eligibility has been determined and/or your library card application has been processed, a staff member will contact you to discuss your reading preferences and discuss delivery times.
